UCSF and UC Davis

Challenge

UCSF and UC Davis won an award from the California Governor’s office to advance precision medicine through the use of personally collected data. UCSF and UC Davis wanted to use remote patient monitoring to understand how changes in medication could be used to titrate tracking of blood pressure in hypertensive patients, and mood in depressed patients.

Solution

Overlap worked with the UC Davis team to get patient demographic and medication list data out of two Epic instances via a FHIR interface. UCSF and UC Davis then used Overlap’s dashboard to onboard these patients, setting up custom tracking by disease domain, and deployed Overlap’s mobile apps on iOS and Android to collect data from different wearables and apps. Once patients were enrolled, the team used the dashboard on an ongoing basis to visualize the collected data, see analytics about enrollment and engagement, get notifications about medication changes, and change tracking in real-time to assess the impact of different medication regimens.
